How to request your child’s records from us, or send records to us.

Requesting and sending records

Records requests go through a signed release form. That applies whether you want us to send your child’s records elsewhere, or you’re bringing records to us from another practice.

Requesting records from us

Complete the Medical Records Release form, listed in the documents on this page, and return it to the office. We need it signed by a parent or legal guardian — we can’t release a child’s records on a phone call, even to a parent we know.

Tell us on the form where the records should go and roughly what you need. “Everything since birth” and “immunizations only” are very different requests, and saying which you want saves a round trip.

Requests for records take time to process. If you have an appointment coming up with a specialist or a new school deadline, send the request as early as you can rather than the week before.

Sending records to us

If you’re joining the practice, we need your child’s complete previous medical and immunization records before their first appointment. Since July 2019 we haven’t been able to register a new patient without both eligible insurance and complete records received by our office.

Complete records should include:

  • Newborn hospital discharge documentation
  • Growth charts
  • Immunization history
  • Office visit notes from previous providers
  • Consult notes from any specialists
