Share feedback about a visit
Our patient survey takes a couple of minutes and asks about your most recent visit: getting an appointment, time spent waiting, and whether you left with your questions answered.
Prefer not to use the form? Tell the front desk on your way out, or call the office and ask to leave feedback.
If something went wrong
A survey is the wrong route for anything urgent or clinical. If you have a concern about your child’s care, call the office and ask to speak with a nurse or the practice manager. That gets looked at properly rather than sitting in a queue.
